Other children will be considered as deemed appropriate by local DSS agencies. The project targets children currently in Residential Treatment Centers or group homes who do not have an identified placement at discharge. Appropriate referrals to this project are children who need a therapeutic level of foster care who have significant behavioral, emotional and/or developmental needs that cannot be served at a lower level of care. Embrace focuses on providing family finding services to identify potential Kinship caregivers, individualized training to identified Kinship caregivers using the Traditions of Caring model, home study, and support and supervision services to include ongoing clinical case management and 24-hour crisis support. Kinship Services- Embrace is a member of the Eastern Region Kinship TFC Collaborative, a collection of private providers with an array of kinship services. Trained foster parents work in collaboration with Embrace Case Managers to provide this level of case management, which is incorporated in the youth’s service plan Case Management services include working with professionals involved in each case such as medical providers, mental health providers, school staff, Department of Social Services, Department of Juvenile Justice, Court Systems, as well as insurance providers to ensure appropriate coverage to receive identified services. ![]() Treatment Foster Care (TFC)- Embrace provides TFC Case Management to youth in treatment foster care and includes treatment planning, monitoring of the care plan, and linkages to other community resources as necessary to address identified needs of the youth.
